Complete each section. Your cadet will need his or her own unique email address. (By policy, any communication with a cadet must have at least two adults included - at no time shall a Sea Cadet adult contact a cadet in private).
For section 5 - the emergency contact must be different from the parental contacts. Do not use either parent as an emergency contact.
For blocks 8.c. and 9.g., leave blank - these will be signed by the CO later.
For block 9.b., leave blank - he/she does not have a USNSCC ID number yet.

Have your doctor complete sections 3 through 9.
If your cadet takes prescription medications, complete each section for those prescription medications. Prescription medications include inhalers for a condition such as asthma or an epinephrine injection device to treat anaphylactic shock. If these are prescribed, your cadet must bring them to drill in the event they are needed.

If your cadet does not takes prescription medications, complete blocks 1.a., 1.b., and 1.c.
In block 7, state "Cadet does not take prescription medications."

Do not list non-prescription medications on this form. Before your cadet attends an advanced training (away from the unit), you will be asked to review cadet records again. Many advanced trainings are prepared to administer non-prescription/over-the-counter medications such as Tylenol, Benadryl, and Neosporin - you do not list those on this form. You will have the opportunity to add any non-prescription medications that are not stocked by the advanced training later (for example eye drops for itchy eyes) and a different on-line form.
Does your cadet need any accommodations under the Americans with Disabilities Act (ADA) - these generally are physical accommodations such as "needs extra time to stretch muscles due to a certain condition." Use of this form is rare, but if you have questions please ask.
If no accommodations are requested, leave these forms blank.
Complete each section
The success of the Sea Cadet program is due to parental involvement. We must have adequate adult staff (both male and female) in order to comply with Sea Cadet policy. If we do not, we cannot hold our drills.
Your involvement is strongly encouraged. There are two ways you can help:
Auxiliary - enroll as an adult and support when you can.
Instructor/Officer - enroll as an adult and commit to attending 75% of annual drills.
Both Auxiliary and Instructors/Officers:
(1) pay a $50 annual enrollment fee;
(2) must take and pass two on-line classes (1-2 hours);
(3) clear a criminal background check;
(4) may participate in advanced training opportunities as staff members;
(5) may take on leadership roles in the Sea Cadet program
Instructors/Officers may wear the Sea Cadet uniform if the member is in compliance with Navy uniform regulations regarding body composition.
